MEDEX Verifications Desk – Information & Instructions
All requests must begin by emailing: medxvrfy@uw.edu
This includes requests intended for the Program Director.
After graduation, the Verifications Desk is the first point of contact for any communication with the Program so we can route your request quickly and accurately.
Graduates: In every email, add your full name [and/or your name as a student], graduation year, and campus & cohort number. (For recent graduates that is Seattle 57 (2025), Spokane 27 (2025), Anchorage 15 (2025), Tacoma 11 (2025), or Kona 04 (2025).
Please email us when your contact information changes. We often need to contact graduates with questions about their verifications.
Employers: Please include your release form signed by your applicant, or ask the applicant to email us permission.

Employers
- For correspondence from the Program Director or confirmation of education, always email medxvrfy@uw.edu first.

- We recognize that requests are time-sensitive and will respond as promptly as possible.
Graduates: Email medxvrfy@uw.edu and request a pre-filled Verification of PA Education form for your state board. We have forms ready for many states.
Guidance by state:
- HI, MT, TN, WA — Only a graduation letter is required; ask us to submit one directly to the boards.
- UT — Only a transcript is required; they no longer use their form.
- For other states, you may download the verification form directly from the medical board website. Rename the file: Lastname_Firstname_StateAbbreviation (XY). Complete only your portion and email the form to us.
- If your state (e.g., TX, NJ) requires a photograph: attach a passport-quality photo to the form, scan the full document in color at the highest resolution available, and email it to us.
- If a state form is sent to us by an employer or credentialing agency, please email us your permission to release information.
- Please do not mail paper forms. All U.S. state boards now accept emailed submissions.
Only the UW Registrar can provide transcripts, including when a state form instructs MEDEX to submit them. Students and graduates may request transcripts here: https://registrar.washington.edu/students/transcripts/
Graduates seeking a letter addressing personal traits should contact a faculty member or preceptor, as they are often the most appropriate source.
If a reference must specifically come from the Program Director, email the request to medxvrfy@uw.edu for review.
- For a scanned copy of the MEDEX Certificate of PA Studies email medxvrfy@uw.edu.
- For a replacement paper copy, you may email the same address.
- MEDEX does not issue the degree diploma; MEDEX issues only the Certificate of PA Studies.
- The degree (BCHS or MCHS) appears on the UW transcript.
- The paper diploma is mailed directly to you by the University.

Replacement diplomas are available for purchase and delivery for graduates by using the Replacement Diploma Request Portal. Diploma website: https://registrar.washington.edu/diplomas/
New Graduates:
The Registrar awards the MCHS degree once per quarter. If your official graduation date is delayed for any reason, the degree will be posted during the next quarter. MEDEX does not control this timing. For verifications in the meantime, we can provide a Graduation Date Letter confirming that you have fulfilled degree requirements. Please email medxsrvc@uw.edu.
Email medxvrfy@uw.edu to request a copy of your Evidence of Professional Liability Coverage showing malpractice coverage during your time in the program.
If an employer requires your personal claims history, they may email their request (with your signed release form) to: rmcerts@uw.edu (UW Compliance and Risk Services)
You may email us at medxvrfy@uw.edu to request a copy of your EXXAT 15-page summary report of clinical encounters.
