To Verify MEDEX Education: Guidance for Graduates

Do you need proof that you completed your MEDEX Certificate of PA Studies? Here are tips for state board licensing, and for credentialing with an employer:

  • Email only medxvrfy@uw.edu with all requests, including forms for the program director.
  • In every email, tell us your campus, class number, and graduation year.
  • Share the email medxvrfy@uw.edu with your future employers as well. Even if they need feedback from the program director specifically, the verifications team must first log and prepare all requests.
  • Give your future employers a signed Release of Information form to include in their email to us. Or, email medxvrfy@uw.edu directly and grant MEDEX permission to tell an employer that you studied with us.
  • If you are applying for a license in a high-destination state (WA, CA, ID, HI…), email medxvrfy@uw.edu and ask MEDEX to email you a pre-filled form. If you can fill this in and email it back IN COLOR, that will speed up the turnaround time.
  • If you need transcripts, please contact the UW Registrar: https://registrar.washington.edu/students/transcripts/. Some state licensing forms specify that the PA Program must provide these; only the UW Registrar has UW transcripts and the authorization to share them.
  • Please email all requests; don’t post-mail any paper forms to MEDEX. Electronic verifications are now accepted by all states, including Texas. (Texas instructions specify that your Form D must be post-mailed to you to include with your application, however it is now emailed directly to them.)
  • If you need a copy of your MEDEX Certificate of PA Studies, we will email a scanned image at your request.
  • If you need to verify your BCHS or MCHS degree, email the UW Registrar:  https://registrar.washington.edu/students/enrollment-and-degree-verification/
  • Please keep in touch: Email medxvrfy@uw.edu with contact information updates; this lets us reach you with any questions. Please also include information about where you are working, which is very helpful for the program to know. We also simply appreciate hearing from you all!
