Family Medicine attracts exceptional faculty and staff who have made remarkable contributions to the department, specialty, and communities over these past 50 years. We are excited to invite you to nominate current or past faculty and staff affiliated with the department for awards to celebrate their achievements and reward their success.

Categories

A total of 50 DFM Awards will be given in the following categories in 2021. Full descriptions of each category are listed under “Descriptions”:

  1. Clinical Service Excellence 
  2. Educational Excellence 
  3. Research & Scholarship Excellence
  4. Leadership Excellence 
  5. Justice, Equity, Diversity, and Inclusion (JEDI) Excellence 
  6. Staff Excellence

The chair of the department will award four Chair Awards; nominations are not accepted for chair awards.

Nominee Characteristics

We want to celebrate the wide diversity of DFM achievement at all levels. To that end, we encourage you to nominate people at all stages of their career – early, mid, late, or retired. Nominees may have focused their work within DFM, in the WWAMI region, or beyond. We value nominations of people who have made big visible impact as well as those who worked behind the scenes to make DFM great. We welcome nominations of individuals as well as teams. To aid our selection committee, you will have the opportunity to select characteristics of your nominations including: Early career; mid career; late career; retired career. Visible impact; behind the scenes impact. Individual recognition; team recognition. Departmental; WWAMI.

We want to celebrate the wide diversity of work across the department and the region at every stage. Selection of characteristics is not required but may provide additional information to selection committee.

Eligibility

Anyone with a current or former relationship with the Department of Family Medicine is eligible. During the 50th anniversary year, former employees, affiliates, trainees, residents, and emeritus faculty are eligible. Nominations should cite work related to the award category description. Nominations must include specific examples of the nominee’s excellence and illustrate the impact that person has made to the DFM. Nominations closed July 7th, 2021.

Selection

Selection of award recipients will be done by a diverse selection committee (e.g. junior and senior staff and faculty, etc.) using a scoring rubric. The rubric for each category is linked below.

Descriptions

Clinical Service Excellence Award

The clinical service excellence award celebrates any individuals or teams that have made significant contributions to clinical care of patients and communities and teamwork with clinical staff and institutions/organizations. Consideration for contributions to health equity and addressing the needs of under-resourced communities will be highlighted in this award as well.

Criteria/Consideration for Clinical Service Excellence Award

Recipients will be chosen based on criteria. Nominee should exemplify many of the listed criteria in their award category, though they are not required to exemplify ALL criteria.

  1. Exceptional medical knowledge and clinical acumen
  2. Exemplary relationship with both patients and clinical staff
  3. Unwavering commitment to care for all patients at all times
  4. Commitment to continued learning
  5. Contributions to health equity and care of under-resourced communities

Educational Excellence Award

The educational excellence award celebrates any individuals or teams that have made significant contributions to teaching, advancement/sponsorship, and mentorship of learners and colleagues. Consideration will be given to nominees who have demonstrated work in curricular development and innovation and those who have made impacts in improving diversity and inclusion of medical/health education fields.

Criteria/Consideration for Educational Excellence Award

Recipients will be chosen based on criteria. Nominee should exemplify many of the listed criteria in their award category, though they are not required to exemplify ALL criteria.

  1. Extensive knowledge and mastery of the subject matter
  2. Innovation in course and curriculum design
  3. Ability to inspire, guide, and mentor students through independent and creative thinking
  4. Serve as a mentor, collaborator, and consultant to other faculty and teaching assistants by helping to enrich the scholarship of teaching and learning

Research & Scholarship Excellence Award

The research and scholarship excellence award celebrates an individual or team that directly contributes to research and scholarship focused on the advancement of education, discovery and innovation.

Criteria/Consideration for Research & Scholarship Excellence Award

Recipients will be chosen based on criteria. Nominee should exemplify many of the listed criteria in their award category, though they are not required to exemplify ALL criteria.

  1. Extensive knowledge and mastery of their subject area
  2. Original research in peer-reviewed journals
  3. Peer-reviewed research presentations (lecture, abstract, panel)
  4. Impactful research discoveries
  5. Ability to inspire, guide and mentor fellow investigators (faculty, fellows, residents, and medical students)
  6. Research support generated through grants and contracts (This criterion does not apply to professional staff scientists or others who may be ineligible to pursue grant funding as a Principal Investigator)
  7. Local, regional and national organizational service relating to research
  8. Mission-driven work

Leadership Excellence Award

The leadership excellence award celebrates individuals or teams that have made significant contributions to advancing the mission and vision of the Department of Family Medicine. Consideration will be given to nominees who have demonstrated commitment to teamwork, sponsorship of others, and contributions to diversity, inclusion and equity within the organization.

Criteria/Consideration for Leadership Excellence Award

Recipients will be chosen based on criteria. Nominee should exemplify many of the listed criteria in their award category, though they are not required to exemplify ALL criteria.

Justice, Equity, Diversity, and Inclusion (JEDI) Excellence Award

The Justice Diversity Equity and Inclusion (JEDI) Excellence award celebrates individuals or teams that have made significant contributions to improving diversity within the DFM community, supporting inclusion and equity within our workplace and communities, and promoting and advocating for justice in our work environment and larger organization.

Criteria/Consideration for JEDI Excellence Award

Recipients will be chosen based on criteria. Nominee should exemplify many of the listed criteria in their award category, though they are not required to exemplify ALL criteria.

  1. Actions and activities that promote an environment that is welcoming, supportive, and nurturing to all
  2. Actions and activities that raise awareness and consciousness about diversity and inclusion across DFM
  3. Advocacy that has had a significant impact on justice, equity, diversity and inclusion with DFM
  4. Publications/presentations that promote JEDI
  5. Serve as a mentor to learners and colleagues by helping with career, leadership, or scholarship development with a focus on enhancing the experience and opportunities available to those historically excluded in medicine and academics.

Staff Excellence Award

The Staff Excellence award celebrates individuals or teams that have made significant contributions to advancing the mission and vision of the DFM, improving staff and faculty morale and cohesion, and leading teams to innovate and serve the needs of our organization, patients, and communities.

Criteria/Consideration for Staff Excellence Award

Recipients will be chosen based on criteria. Nominee should exemplify many of the listed criteria in their award category, though they are not required to exemplify ALL criteria.

  1. Outstanding job performance that exceeds expectations for position
  2. Creativity and innovation
