Thank you for presenting at Family Medicine Grand Rounds. These presentations are an opportunity to highlight the breadth and depth of activities in our department.
Time and Place
Family Medicine Grand Rounds is held regularly on the first Wednesday of each month at 12:00-1:00 PM. Currently all presentations are held at:

MEDEX conference room 202 in R1
AV Equipment
We routinely provide a laptop Mac computer, projector, screen and slide controller/laser pointer. If you need other equipment, please let us know in advance.
CME
The Department of Family Medicine sponsors CME credits for all physicians and physician assistants who attend Family Medicine Grand Rounds. To qualify for AAFP Prescribed Credits, each grand rounds presentation must include the following four items. We must have this information included in your slides to qualify for CME credit. Please let us know if you have questions.
Learning Objectives
In an opening slide, you should briefly list a few (3-5) learning objectives for your presentation. These are usually best stated in behavioral terms, for example:
By the end of the activity, the participant will be able to:

2. Describe….
3. Identify…
4. Apply….

The form covers two essential disclosure statements:
Disclosure Forms
Disclosure of conflict of Interest.
Complete the attached form on disclosure of potential conflicts of interest with any drug or product mentioned in your presentation. We need one of these statements from each person who presents or is listed as an author on the title of the presentation. See the full text of AAFP policy.
Disclosure of Off-Label Drug Use.
Each presentation needs one statement about any discussion of off-label or investigational uses of drugs, devices and/or products mentioned in the presentation. (This doesn’t apply if you do not discuss any products; in that case, “just say no.”) See the full text of AAFP policy.

Archiving.
We are also archiving all Family Medicine Grand Rounds presentations. For now, that means posting a copy of your PowerPoint slide set on our grand Rounds website. In the future, it may mean posting videos of the entire presentations. Please send us a copy of your PowerPoint slide set. We will be sure it includes the required CME notice and disclosures. We recommend you add a footer noting a copyright in your name (©2015 Name). We will post the slide set online as a locked Adobe PDF document, so that people can view it online but they will not be able to download, change or use your slides. We understand that your slides are not your presentation, but some viewers will find them helpful.
Essentials for presenters:
1. Confirm the title and date of your presentation.
2. Complete and return the Disclosures form by email.
3. Add the boilerplate slides into your PowerPoint slide set.
4. Add copyright note in a footer on all your slides.
5. Send us a copy of your completed PowerPoint slide set.
