The Healthier Washington Practice Transformation Support Hub Resource is launching Version 1.2 of The Portal on August 31st! The V1.2 launch will include a new easier-to-use revised Resource page, updated resources, and the interactive Practice Transformation Directory. The design and the curation of resources on this web portal is an iterative process, driven by end user feedback and user analytics. Our mission is to help primary care practices and community health centers move from volume to value-based reimbursement, navigate practice transformation to improve patient outcomes, and integrate physical and behavioral health. The Portal is a PCI-Labs project funded by the Washington State Department of Health (DOH), the Health Care Authority (HCA), and the Center of Medicare and Medicaid Innovation (CMMI).
