{"id":80,"date":"2018-02-27T04:42:56","date_gmt":"2018-02-27T04:42:56","guid":{"rendered":"http:\/\/www.depts.washington.edu\/fammed\/improvingopioidcare\/?page_id=80"},"modified":"2021-05-28T00:01:19","modified_gmt":"2021-05-28T00:01:19","slug":"tracking-monitoring-patient-care","status":"publish","type":"page","link":"https:\/\/familymedicine.uw.edu\/improvingopioidcare\/6-building-blocks\/tracking-monitoring-patient-care\/","title":{"rendered":"Tracking &amp; monitoring patient care"},"content":{"rendered":"<h2>Why is this important?<\/h2>\n<p>Identifying who is on long-term opioid therapy for their chronic pain is important for three reasons: 1) this is an \u201cat-risk\u201d population so identifying who they are provides both an opportunity to risk-stratify them and also produce reports about them so that they don\u2019t \u201cfall between the cracks\u201d of a busy primary care clinic; 2) a population tracking system can be used to identify care gaps between scheduled visits and conduct follow-up with those patients; and 3) population tracking provides an opportunity to track if efforts to improve care are successful.<br \/>\n<div class=\"box\" \/>XYZ Medical Center has chronic pain management team, comprised of a nurse, pharmacist, and prescriber, that identifies and monitors patients on high opioid doses and offered specific recommendations to help providers decide how to make changes.<\/div>\n<h2>Example activities<\/h2>\n<ul>\n<li>Produce a report of current patients on long-term opioid therapy<\/li>\n<li>Identify data and measures needed for pre-visit planning and monitoring success<\/li>\n<\/ul>\n<p>&nbsp;<br \/>\n&nbsp;<br \/>\n<a href=\"https:\/\/familymedicine.uw.edu\/improvingopioidcare\/self-assessment\/tracking-and-monitoring-patient-care\/\" class=\"btn btn-primary btn-lg\">Self-Assessment Questions<\/a>\n<a href=\"https:\/\/familymedicine.uw.edu\/improvingopioidcare\/helpful-resources\/resources-for-clinics#tracking\" class=\"btn btn-success btn-lg\">Resources<\/a>\n","protected":false},"excerpt":{"rendered":"<p>Why is this important?
